Documentation from the family and educator confirmed the educator tickled a child with her finger to gain attention. This behavior does not align with expectations for modeling appropriate social interactions and maintaining professional boundaries. Tickling is not considered appropriate physical contact in a childcare setting, as it can be misinterpreted and does not support respect for personal space. Immediate Action: On the date of the complaint, the educator was placed on administrative leave pending the results of the investigation. Preventative Action: To prevent recurrence of inappropriate interactions between staff, children and peers, the director held a mandatory all staff meeting on October 28. 2025 where such topics were discussed. All staff that attended the meeting were required to sign in and that was provided to the licensing compliance officer for review. The director will continue to monitor staff for appropriate interactions and will provide additional guidance where needed. The rubber mats on the infant/toddler playground (yellow canopy) are not safe in that the corners of the mats are lifting causing a potential tripping hazard. 1. On 06/11/2024, the Director talked to maintenance to get them to repair or replace the rubber mats. 2. Effective 06/11/2024 the Director or delegated person will inspect the playground for potential hazards including the ground and flooring. 3. Effective 06/11/2024 the director will do a help desk ticket to the maintenance department who is normally at the Center the next business day to make any repairs that are noticed during the playground inspections. 8.9.4.24 · H SOCIAL-EMOTIONAL RESPONSIVE ENVIRONMENT
Educators in the 2 yr. old room(s) are not actively engaged with children when one Educator went to retrieve the emergency backpack for the recess break and was unavailable for longer than should have been necessary to retrieve a backpack. The Educator was supposed to be helping children get ready to go outside while another Educator in the room was assisting a child with their jacket. During this time two children were able to open the back door to the classroom and get a head start onto the playground without supervision for approximately 10 seconds. On 01/02/2024 all Educators in the facility will be attending a staff meeting where additional training on supervision, guidance and discipline will be presented. Name to face procedures will also be reviewed. Bells were installed onto each door in the classroom. This will allow educators to hear when the doors are opened. The Center will obtain safety locks on the exit doors, so they are not easily opened by children but still maintain the requirements for easy exit in an emergency situation.
