8.9.4.22 · F PERSONNEL RECORDS
From the review of staff records, it was determined that 1 out of 2 staff records does/do not include a background check. See Staff Records 8.9.4.22 form for staff with this missing information. Immediate Action: On 03/11/2026, staff was removed and a staff member with an appropriate clearance was issued to the site for coverage. The manager also emailed BCU to get additional clearance. Preventative Action: On 03/11/2026, the CS Manager sent an additional list to BCU to get additional clearance for sister sites. The manager also created staff files to maintain within each applicable center. Compliance Monitoring: Starting on 03/15/2026, the center supervisor/teacher II will do a monthly file check to make sure all required documents are at the correct location for coverage for staff. 8.9.4.22.F.1.:A licensee shall keep and maintain a complete up-to-date file in ECECD’s professional development information system (PDIS), for each staff member, including substitutes and volunteers working more than six hours of any week and having direct contact with the children. If the center chooses to retain duplicate physical copies of these records, these records shall be stored in a secure location for the privacy of the staff members. New information shall be updated in a timely manner in PDIS. Providers will have six months to comply from the date these regulations are promulgated. Licensing observed a gap under the east side playground gate that could allow children to exit the playground area. Immediate Action: On 03/12/2026 CS Maintenance completed a work ticket to correct the gap under the east side gate. Additional dirt was added to fill in the gap. Preventative Action: As of 03/12/2026, the site supervisor will do daily outdoor walks and submit a work ticket with any issues that could cause a potential hazard. 8.9.4.24 · H SOCIAL-EMOTIONAL RESPONSIVE ENVIRONMENT
During the inspection, it was observed that the educator(s) in the Head Start room(s) was/were not calm when a stressful situation occurred when one child was not doing as the educator asked when the educator responded " (Child's name) if I have to tell you again, I will throw your breakfast in the trash!" It was also observed that the educator again raised her voice at the same child during a separate interaction. The center does not provide appropriate storage for toothbrushes as evidenced by toothbrushes are in contact with another surface. There was a toothbrush observed laying on the counter next to the sink. Immediate Action: ON 2/13/2025 the Director reminded teachers to put the toothbrush in its proper place. Preventative Action: On 2/13/2025 the Director will remind educators to put toothbrushes away immediately after brushing their teeth. Educators did not complete the following training within 3-months: Health and Safety Training, Covid-19 training. 1, As of 09/04/2024, the educator has completed all required trainings. The educator is waiting on one certificate. 2. As of 08/26/2024 the Director will double check with staff members after they have been assigned new training courses to make sure they are completed. 3. As of 08/26/2024, the Director will continue with the compliance monitoring they already have in place but will monitor with more accuracy. This is an ongoing effort. The center’s first aid kit does not contain soap, adhesive tape. 1. On 08/15/2024, the Director replaced the soap and adhesive tape in the first aid kit. 2. As of 08/15/2024, the Director and educators will check the first aid kit supplies but with more accuracy. 3. As of 08/15/2024, the Director said there is a compliance monitoring system in place with the use of a checklist form.
