The licensee contracted with a hay ride company, knowing the owner was a former educator who had her background clearance revoked and was terminated from the center. Based on the revocation of the background check, the individual was to not be around children. Immediate Action: Develop a policy regarding former employees performing on-site services. Preventative action: Implement the new policy Compliance monitoring: Director will monitor vendor payment requests. Any new or unrecognized vendor name will be flagged for discussion with the center director or administrator. Any director or administrator not in compliance with the policy will be disciplined according to the policy.

The premises in the kitchen are not clean as evidenced by mouse droppings found under the food shelf and behind fridge. On 3/14/24 director will clean and disinfect the droppings in the kitchen immediately. Director will also contact their pest control company to do a check up on the situation. On 3/14/24 director and co-director will create a checklist to ensure the cleaning and sanitizing is being done daily. On 4/1/24 director will have a monthly staff meeting to go over updates and will include a deep clean while children are not present at the daycare.
