Of the 12 children's records reviewed, 1 is/are missing a list of people authorized to pick up the child and an authorization form signed by the parent or guardian. See Children's Records 8.9.4.32 D(1)(e) form for the child(ren) with missing information and/or authorization. Immediate: Contact the parent/guardian of the identified child immediately to obtain a complete and signed authorization form for pick-up, including a comprehensive list of authorized individuals. Preventative: Schedule an annual review of all children's records to ensure completeness and accuracy of information. Compliance Monitoring: Randomly select and review 1-2 children's records monthly to ensure ongoing compliance with documentation requirements.

Educators are not onsite, available and responsive to children during all hours of operation as evidenced by secondary employee on her cell phone during outdoor playtime with the children. Immediate-Specialist informed primary caregiver regarding the concern and spoke to employee.
