Electrical outlets within reach of children in the Infant classroom & Hallway near playground are not safety outlets and they do not have protective covers. Immediate - Center replaced outlet covers the same day of inspection. Corrected on site. Preventative -Eff 2/3/2026 Director will include outlet covers in weekly checklists for educators. Monitoring - Director will review checklists monthly to ensure forms are being completed and outlet covers replaced timely.

Adults are not able to visually supervise all centers at all times in the Preschool class room(s) due to furniture or shelving that obstructs the view. The director has already rearranged furniture and took shelves out for a more open space to be able to view children at all times, regardless of where they are in the classroom.
