Immediate: Called parent to request updated immunization records for the child age 6 Preventative: Will ensure to have parents provide me with up to date immunizations after every wellness visit they have. Corrective Action Plan Immediate: Called parent to ensure enrollment form is completed by pick up this afternoon. Preventative: Will ensure to have parents complete enrollment forms prior to starting child care. Compliance Monitoring: Will review records quarterly to ensure documentation is completed correctly.
