Educators did not complete the following training within 3-months: Health and Safety Training Immediate Action: On 4/4/2025, the director will obtain the training certificates for employee files and place them in the file. Preventative Action: On or after 4/4/2025, the Director will ensure that all "shadow" files for employees contain all the pertinent information in each staff file. The files will mimic the main file. Compliance Monitoring: As of 4/4/2025, the Director will check all staff files in August, December and May to ensure they are up to date.

A food, drink is not properly stored; the item is not labeled and dated. In the refrigerator there was a container of milk not in the original container that was not labeled or dated. There were also cut up cucumbers in a container with no label or date attached. Immediate Action: On 4/4/2025, the Director will have the program aid will label and date all open containers in the refrigerator. Preventative Action: On 4/4/2025 the Director will post a reminder note on the door of the refrigerator reminding staff to label and date all open food. Compliance Monitoring: As of 4/4/2025, the Director will monitor the refrigerator monthly to ensure all items are labeled and dated.

8.9.4.24 · J OUTDOOR PLAY AREAS
The fall zone underneath the yellow twist slide is not adequate as evidenced by the energy absorbent material is not uniformly spread over the fall zone area. As of 03/20/2024 the Director has ordered additional woodchips to be added to the playground. The Director will go out on the playground and spread the wood chips out more uniformly. On 03/20/2024 the Director will consult with maintenance to double check the playground list and add that to the list if it is not currently on there. The maintenance supervisor will review the playground checklist to make sure all items are being inspected. The Director will touch base the supervisor on a monthly basis to ensure that this is being done. As of 03/20/2024 the Director will check with the maintenance supervisor to make sure the materials have been ordered and the checklists are being completed. 8.9.4.24 K SWIMMING, WADING AND WATER N/A
