On 6/18/25 the center modified their accident report form to include a spot for contacting ECECD in the event that a child feels threatened. This will allow center to accurately track on the accident form if and when ECECD is contacted both by phone and when a follow-up email is made, as well as who was spoke to. The playhouse where the incident took place will now be off limits to children and will be removed from the playground when the trash is picked up. Center updated Cell Phone Policy to ensure that teachers are remaining focused on the children and not distracted by personal devices. Each room has their own tablet that can be utilized for Procare so that personal cell phones can be placed in the office or personal bags and can only be used during breaks, lunch, and nap time.

The facility will secure sand and delivery to the playground. The emergency exit door in the 2-year-old classroom does not latch. During the licensing visit the door was blocked on the exterior by plastic crates that prevented the door from opening. The Director did remove the crates, but the door does not latch and will not stay closed which presents safety issues. Immediate Action: On 1/21/2025, it was alerted to the Director that the door in room two would not open. A maintenance man was called and scheduled to come the following day (Friday morning). Preventative Action: On 2/1/2025 the maintenance man determined the cause, bought the necessary items to fix the door and returned Friday evening at closing time to complete the repair. The door now opens and closes properly. Staff will continue to monitor the exit doors to ensure they remain in good working condition. 8.9.4.29 · H SAFETY COMPLIANCE
The center does not have verification of an annual fire inspection from the fire authority having jurisdiction. Director was not in during the visit, and assistant director is fairly new. Spoke with Director over the phone and she stated the fire inspection is somewhere because she did had to scan it in, it just got misplaced but they have it. Director stated it will get sent in within the next 24 hours.

The premises in the classroom 3 are not safe in that a outlet is exposed and needs to be checked. Will follow up with maintenance. Will add to checklist. Outdoor Play 8.9.4.29.A.1.:A center will keep the premises, including furniture, fixtures, floors, drinking fountains, toys and equipment clean, safe, and in good repair. The center and premises will be free of debris and potential hazards. Finding The equipment are not in good repair as evidenced by the red slide is cracked.
