See Children's Records 8.9.4.32 D(1)(e) form for the child(ren) with no immunization/exemption. Immediate: The Provider has requested a copy of up-to-date immunization records for the child. This will be received by 12/6/25. Preventative: Effective 11/6/25, the Provider will review immunization records every 3 months to ensure all children's records are up-to-date. Monitoring: The Provider will create a calendar reminder to review the children's records quarterly.
