Home educators do not have a signed statement that they would or would not be disqualified as a direct provider of care under the most current version of the Background Checks and Employment History Verification provisions pursuant to 8.8.3 NMAC. Immediate: On 7/28/2025 provider will have all staff sign annual abuse statement. Preventative: On 7/28/2025 provider will set a reminder on her calendar for July to sign all statements. Monitoring: On 7/28/2025 provider will be responsible to check all staff files on a quarterly basis.

The home failed to keep a training log on file with date of hire, position, date of training, employee's name, clock hours, competency area, source of training, training certificate for 4 out of 4 staff.
