A child was observed sleeping in a pack and play; the child was not removed to a crib. On 1/16/2015 provider corrected this on site. Provider removed the child from the pack and play and was moved to a crib. Ongoing, provider stated she will only allow the child to play in the pack and play and remove him on to the crib when child falls asleep.

On 1/16/2024 the provider stated she would move crib and shelf to make the emergency exit accessible. On or before 2/14/2025 provider will move the shelf over to let the door be accessible. There was no record of each child's arrival and departure time and dates of attendance initialed by a parent, guardian, or person authorized to pick up the child. Of 4 children present 0 were signed in. See Children's Records 8.9.4.32 D(1)(e) form for the child(ren) with missing information. On 11/06/24, provider will immediately have parents of children sign them in and out for today. Provider will send proof to department via email or text message. On 11/06/24, provider will move sign in sheet by front door so that parents have easy access to it. Provider will make sure parents sign children in and out daily to include date, time and signature or initials. Provider will also make sure children are signed out if they are taken to school.
