Home educators do not have a signed statement that they would or would not be disqualified as a direct provider of care under the most current version of the Background Checks and Employment History Verification provisions pursuant to 8.8.3 NMAC. 12/18/2024 Provider will print new form and have secondary provider sign the statement. Ongoing, provider will set a calendar reminder to sign the statement every November, before the renewal is conducted. Finding The home does not have a written plan for ongoing professional development for each staff member, including the director, that is based on the seven areas of competency, consistent with the career lattice, and based on the individual's goals. Corrective Action Plan On 12/18/2024 provider will work on completing the plan for employee development for herself and her secondary provider. Ongoing provider will use the checklist provided by licensing to ensure the plan is maintained and kept up to date.

8.9.4.33 · B Staff Qualifications and Training

The home failed to keep a training log on file with date of hire, position, date of training, employee's name, clock hours, competency area, source of training, training certificate for 2 out of 2 staff. See Staff Records 8.9.4.33B(3) form for staff who are missing a complete training log. On 12/18/2024 provider will print a new training log for herself and her secondary provider. Ongoing provider will set a calendar reminder every three months to check the training logs and ensure it is kept up to date. Finding The home does not have all educators certified in first aid and cardiopulmonary resuscitation (CPR) with a pediatric component. Corrective Action Plan On 12/18/2024 provider will schedule an appointment for secondary provider to take the first aid and CPR class by or before 1/17/2025. Provider will check the first aid and CPR class quarterly to ensure files are kept up to date, avoiding having expired first aid and CPR certificates. Services & Care of Children

The home's policies and procedures regarding the following need to be included: emergency procedures, safety policies, disaster preparedness plan, and plan for care during a public health emergency.
