By evidence of front room at 86 degrees Fahrenheit, and the back room at 90 degrees Fahrenheit. Immediate: Provider corrected this on site, provider turned on Central air. The Fans, vents, walls/doors, classroom equipment, refrigerator, and floors including bathroom has a heavy accumulation of dust, dirt, spills and cobwebs.. On 7/10/2024 provider will dust all areas and is going to paint the classrooms. Every week during nap time provider will clean classroom equipment. Provider will create a checklist to endure classroom is kept clean. Finding The premises are not in good repair as evidenced by broken items/trash on playground, storage and clutter on playground/patio, and mats ripped/torn. Corrective Action Plan On 7/10/2024 provider will clean the playground and dispose of broken items and trash. Provider will no longer utilize the patio area; provider will order a shade for the playground area. Provider will dispose of the ripped/torn mats and order new ones. On 7/10/2024 Provider will use tape to cover the slide to prevent an injury. Provider will have a maintenance inspection to determine if slide can be repaired.
