The home does not have a written plan for ongoing professional development for each staff member, including the director, that is based on the seven areas of competency, consistent with the career lattice, and based on the individual's goals. On 1/7/2025 provider will print a new professional development plan and will update it with the information for herself and her staff. Ongoing provider will set a calendar reminder yearly to complete a new professional development plan.

The home failed to keep a training log on file with date of hire, position, date of training, employee's name, clock hours, competency area, source of training, training certificate for 2 out of 2 staff. See Staff Records 8.9.4.33B(3) form for staff who are missing a complete training log. On 1/7/2025 provider will print a new training log for herself and her staff. Ongoing provider will ensure, to write all trainings in log immediately after taking the class to ensure it is written. Provider will submit corrections on or before 2/7/2025 to licensing for review.
