8.9.4.21 · C INCIDENT REPORTING REQUIREMENTS
The center failed to notify parents or guardians in writing of an incident regarding an injury that occurred at the center. Immediate Correction: On January 19, 2026, The Director reviewed the incident reporting requirements with all staff during our staff meeting. Staff were reminded that incident reports must be completed at the time of the incident, before the child leaves for the day. Parents may be asked to wait briefly to ensure the report is completed and signed within the required timeframe. Preventative Measures: Effective Immediately, each staff member received clear, written, step-by-step instructions for completing an incident report. The Director clarified expectations by reviewing the licensing requirements in section 8.9.4.21.C.2, all reportable incidents require written documentation, regardless of parent status or future enrollment. Administrative staff and Lead Teachers will oversee and verify that classroom staff complete incident reports accurately and on time. Compliance Monitoring: Effective immediately, the Director and administrative team will review incident reports daily to ensure completion and parent signatures within 24 hours. The vent has a heavy accumulation of dust. Immediate: The Director cleaned the vent immediately. And she reminded the staff to clean the vents weekly.
