IMMEDIATE ACTIONS: Corrections were made on Jan 13th, 2025, the Director has ensured enough wood ships are placed in the playground area placing sufficient energy absorbing surfaces beneath climbing structures and slides. Preventative Actions: All staff will be notified about changes and new procedures in the playground and make sure everybody cleans, uses tools to level the wood chips, and makes sure there is enough material above the surface to prevent falling from playground equipment that can result in a life-threatening injury to the head or body. The Director will make sure to have adequate protective surfacing beneath and surrounding playground equipment. The Director will make sure this issue is addressed as soon as possible.
