The child(ren) in the 3 yr. old classroom(s) was/were left unattended when child walked into the bathroom and through to the four-year-old classroom. The child was unattended without adult supervision for 12 minutes. Immediate action: Assistant Director wrote up two staff for lack of completing name to face upon entry of the class. Preventative action: On 12/13/24 the management will check that staff perform name to face, count the children on the tablet and count the children in the classroom. Then ensure the educators write the number of children on the whiteboard as children arrive and leave for the day. The assistant director will monitor the name to face and whiteboards indefinitely. Monitoring action: The Assistant Director began monitoring the classrooms on 12/13/24 and will check the name to face twice a week.
