Provider stated she was unaware of sign in/out log. There were no children present, however, provider stated children were present earlier in the day before school. The educator does not have the following health and safety training: building and physical premises safety, including identification of and protection from hazards that can cause bodily injury such as electrical hazards, bodies of water, and vehicular traffic, precautions in transporting children, a child development course that addresses all major domains of child development, including cognitive, social emotional, physical development and approach to learning as defined by the federal Child Care and Development Fund in 45 CFR § 98.44 Training and Professional development, or approved three-credit early care and education course or an equivalent approved by the department. 02/17/2026 Provider did not complete Health and Safety training within three months of hire date.
