Educators did not complete the following training within 3-months: Health and Safety Training ( 1 out of 18 Staff), child development course that addresses all major domains of child development ( 1 out of 18 Staff) Immediate -Center will print educators certificates for staff file on or by 11/21/2025. The premises are not in good repair as evidenced by Light bulbs are out in Yellow and Red Rooms.. Immediate -Center will submit a work order to have lightbulbs replaced by 10/31/2025. Preventative -effective 10/22/2025 center will have educators check for working light bulbs weekly. (Classrooms have lots of natural light and bulbs arent turned on often). Monitoring - Facility staff will also check monthly for lightbulbs that are out.

Immediate - Center Director will submit a work order to have bulbs replaced on or by 5/24/2025 Preventative - Center will include light bulbs in classroom inspection log. Monitoring - Center will test and document bulbs once monthly to ensure all bulbs are working. (Centers classrooms have a lot of natural lighting and lights arent use as much)
