A child was observed sleeping in a bouncer/Swing; the child was not removed to a crib. 5/19/25 CORRECTED ON SITE. The educator placed the two infants in their cribs. Preventative: On 5/23/25 the director will ensure educators place infants in cribs after falling asleep. Monitoring: The director will observe infant classroom daily through August to ensure infants are placed in cribs while sleeping. She will schedule in her phone a daily reminder to monitor daily. 8.9.4.24.C.2.:Cribs will meet federal standards and be kept in good repair. The center will not use plastic bags or lightweight plastic sheeting to cover a mattress and will not use pillows in cribs. Stacking cribs is prohibited. Cribs will not be used for storage. Animals and pets will not be allowed in cribs or on sleeping materials. Finding The center does not keep evacuation cribs clean. Corrective Action Plan Immediate: 5/19/25 CORRECTED ON SITE. The educators removed items stored in the evacuation cribs. Preventative: On 5/23/25 the director will ensure that the evacuation cribs are clean, and items are not stored in them. Monitoring: On 5/23/25 the director will check the evacuation cribs weekly through August 2025 to ensure the crib don't have stored items and set an reminder on her calendar.

Feeding practices are inappropriate as evidenced by bottles are propped. Immediate Action: on 1/21/25 the administration gave a Staff Memo to Infant/Toddler staff and posted a visual notice of Bottle-Feeding Procedures. Preventative: On 1/21/25 the administration developed a Bottle-Feeding Procedure. Infant/Toddler staff will be enrolled and complete Infant/Toddler feeding training. Monitoring: The administration will monitor the Infant/Toddler classroom hourly during ratio check to ensure that the feeding policy is being followed. They will add a daily reminder to their phone. The director provided documents of the Infant/Toddler feeding policy, memo and staff sign-in and training.
