8.9.4.21 · B CAPACITY OF CENTERS
It was observed that children were cared for in an unlicensed area. Immediate Corrective Actions: 1. Licensed Space Enforcement All unlicensed areas have been clearly identified. All exit and access doors leading to unlicensed areas have been posted with signage stating they are not to be used. Staff were notified the day of the incident that these areas are prohibited for child use. 2. Supervision & Ratio Compliance Teachers are being monitored to ensure ratios are maintained at all times through the following actions: Name- to-face attendance checks every 30 minutes. 8.9.4.23 · A PERSONNEL AND STAFFING REQUIREMENTS
The child(ren) in the 3 yr. old classroom room(s) was/were left unattended when educators took the children outside to the playground, and one child was left in the bathroom for approximately ten minutes.. Immediate: On 4/23/25 the director self-reported the incident. The child was found by a parent and taken to the educator. The family was informed of the incident. The director met with the educators on the severity of the incident and emphasizing the child's safety not upheld. Preventative: 4/23/25 the director met with staff on the transition process to complete transitions fully and the count children before leaving the area. Educators will perform name-to-face counts every hour using the ProCare system. Ratios will be verified and documented on a whiteboard in each classroom. Educators will communicate ratios with one another during transitions, shift changes and breaks using the stack or whiteboard system. Mandatory room counts will be conducted before leaving any area to count and confirm that all children are accounted for. 8.9.4.23 · A PERSONNEL AND STAFFING REQUIREMENTS
The child(ren) in the 2 yr. old classroom(s) was/were left unattended when the children were transitioning from one class to another. Child was left unattended for less than 2 minutes. Immediate Action (May 31, 2024, and ongoing) Teachers will be checking ratio and counting the children as they go through transitions throughout the day, and counting the children over and over in the class they are in. The Director of Children’s Promise Centers Alvarado will do count checks throughout the day and make sure all teachers know how many children are always in their care. Preventative Actions (Beginning May 31, ongoing) The director will be doing random observations through each class transition to make sure the children are being counted all the time when transition happens. She will also be showing examples to classrooms of what they need to be doing in each room so that way they are being modeled on what to do. We will be reminded weekly through the communication app SLACK. When having our staff meeting, the director will also remind staff about ratio and counting the children throughout the day. Staff will be reminded of the policies outlined in the employee handbook*. Beginning May 31st and ongoing, Compliance Monitoring will be done by the Director daily. The Director will be making sure all classrooms follow ratio guidelines from the ages 6 weeks to 12 years old. The Director will be randomly checking the numbers of each classroom, so they are being reminded throughout the day to keep counting their children in their care. Will not have an end date, due to this be ongoing. To get to where we need to be with ratio, counting, attendance, and more will be done with more understanding with training by end of June 2024. The Director will make sure daily that the teachers are practicing ratio counting and making sure they know where each child is always. By doing this I will be going to each classroom to get numbers and ask them what their ratio is. We are going to keep an open communication to remind one another of how many students each teacher has and to keep going forward with counting each child through transitions.
