The center failed to make a report to the licensing authority within 24 hours after the incident occurred regarding an injury that required medical care beyond on-site first aid, a serious injury. Immediate Action: Staff were trained on 6/19/2024 to alert managers when injuries occur for assessment by managing staff members. Managing staff members reviewed first aid protocols and reminded to call and report injuries if in doubt.
