The center failed to make a report to the licensing authority within 24 hours after the incident occurred regarding an injury that required medical care beyond on-site first aid, a serious injury. Immediate Action: Staff were trained on 6/19/2024 to alert managers when injuries occur for assessment by managing staff members. Managing staff members reviewed first aid protocols and reminded to call and report injuries if in doubt. Preventative Action: On 6/22/2024 managers met and came up with a better method for submission and review of all incident reports on a daily basis. New incident reports were created and staff were informed of new reports and new submission of reports daily.
