8.9.4.23 · A PERSONNEL AND STAFFING REQUIREMENTS
The educator does not demonstrate the ability to perform essential job functions that reasonably ensure the safety 03/31/2026 http://www.nmececd.org 2 of 5 Albuquerque Nursery School 4000920 Personnel (continued) of children in care. (Educators leaving children unattended on changing tables.) Immediate - On March 7, 2026, Center held a mandatory staff meeting in which we discussed the proper procedure for diaper changing. The staff meeting was held in the 2’s class to properly demonstrate the procedure. All staff members were given a printed copy of the diaper changing procedure. A signed copy of this document was included in all staff member’s file. Preventative - Eff 3/7/2026 Center has added the document on our diaper changing policy to our new employee packet. We have also ensured that all rooms that change diapers have a copy posted in the diaper changing area. Some staff members were aware of this situation occurring but had not brought it to the attention of the Director. Within our new employee packet, all staff members sign a form that states: I will immediately report to the Director any situation concerning personnel, students, parents, ore the premises that may be hazardous or detrimental to the school and/or its students or staff. All staff members were reminded of this statement that they agreed to abide by when hired. Staff signed a second copy of this statement. It was discussed that their commitment to keeping the children safe, includes bringing any issues that may be harmful to the Director’s attention. It was discussed that the Director cannot correct issues if the Director is not aware that they are occurring Monitoring - Office staff will make periodic checks within the classrooms to ensure that all steps of our diaper changing policy are being properly followed. We will document these reviews and keep in a file in the front office.

8.9.4.24 · A GUIDANCE
Children were lifted by the arms. Immediate - On March 7, 2026, we held a mandatory staff meeting in which we discussed the proper way to pick up a child. This included only picking up a child with both hands on their torso, never by their arms. Within this discussion we also reviewed Nursemaid’s Elbow and it’s causes. Preventative - Eff 3/7/2026 All staff members were given a document on Nursemaid’s Elbow after we reviewed this condition in detail during our staff meeting. A signed copy of this document has been included in each staff member’s file. This document has also been included to our new employee packet to ensure all staff members are aware of the importance of properly picking up a child and the consequences that could result if not done properly. Some staff members were aware of this situation occurring but had not brought it to the attention of the Director. Water coming from a faucet is above 110 degrees Fahrenheit. Immediate: The Director will place a work order today 11/10/25. The Director will request for water temperature to be turned down. Preventative: Effective immediately all classrooms will check water temperature weekly. Monitoring: Effective immediately the Director will check water temperature monthly.

The refrigerator in the kitchen does not maintain a temperature of 41 degrees (Fahrenheit) or below. The refrigerator observed to be at 44 degrees. On 9/27/24 the Director will place a work order to have the refrigerator serviced. Effective 9/27/24 the Director will do a maintenance checklist weekly to ensure everything is in working order. 8.9.4.29 · A HOUSEKEEPING
The premises are not in good repair as evidenced by handle missing for the cold water in the infant room. On 9/27/24 the Director will place a work order to have the sink fixed over the weekend. Effective 9/27/24 the Director will do a maintenance checklist weekly to ensure everything is in working order. Effective 9/27/24 office staff will do quarterly checks to ensure the weekly maintenance checklist are being completed.
