Of the four children's records reviewed, One is missing a copy of an up-to-date immunization record or public health division approved exemption. See Children's Records 8.9.4.32 D(1)(e) form for the child(ren) with no immunization/exemption. 04/09/2025 Provider failed to obtain updated immunization record for one child in her care. Immediate Action: 04/09/2025 Provider will request updated immunization record from parent when child returns to daycare. Preventative Action: 04/09/2025 Provider will ask for updated immunization records from all parents during open enrollment in December 2025. Compliance Monitoring: 04/09/2025 Provider wrote in notes to update immunization records during open enrollment in December of 2025. Finding Of the four children's records reviewed, four are missing written authorization, containing fieldtrip destination, date and time of fieldtrip and expected return time, from the child's parent or guardian to remove the child from the premises to participate in off-site activities. Corrective Action Plan 04/09/2025 Provider failed to obtain permission to transport from all parents. Immediate Action: 04/09/2025 Provider will get permission to transport from all parents by next week. Preventative Action: 04/09/2025 Provider stated she is now aware of permission to transport form she will add it to enrollment packet.
