The child(ren) in the 3 yr. old preschool room(s) was/were left unattended when child went to restroom by herself.. Immediate - Director will ensure all children are accompanied by a staff member when going to the restroom. There was a misunderstanding with director walking licensing to check the vehicles and child followed director out of the classroom. Preventative - Classroom will go to the restroom all at once as a group. Monitoring - Director and educator will communicate via text/talk to ensure educators are present when children are going to the restrooms.
