The child(ren) in the 3 yr. old class room(s) was/were left unattended when using the restroom after coming in from the playground, a child ran away from the restroom and down the small hallway to the front door. Director was in a closed door meeting with Compliance Officer, but immediately opened the door upon hearing the educator call for the child. Child was out of line of sight for the time it took for him to run the small hallway until the Director opened the door.. IMMEDIATE: Director will rearrange classroom combinations so that children who are prone to running will be primarily in classrooms with an attached restroom. Director will also instruct educators on keeping children prone to running closer to the educators during transitions. PREVENTATIVE: Director has already purchased baby gates and will install once they arrive. MONITORING: Director will continue walking the center daily and will conduct walks during transition times.

Educator(s) failed to make appropriate physical contact to comfort and support children in daily routines and interactions when children were distressed. The following was observed: a child was running around the class and appeared to run into a table at which point she began crying. The educator turned and yelled at the child that she had told her and told her not to run in the classroom and ordered the child to get in line. Video observation revealed an educator forcibly pulling a child up from the floor by his hands, continuing to hold a child's hand after the child stopped standing up resulting in the child dangling from his hand, as well as continuing to hold a child's hand even as the child attempted to yank his hand from the educator's hold. IMMEDIATE: Director placed the educator in question on suspension immediately upon being notified by parent. The facility’s outdoor play area is not adequately enclosed as evidenced by the latch on the gate is broken. The latch in the larger playground needs lubrication or additional repair to ensure closure. IMMEDIATE: Director will lubricate the latch today. The furniture are not in good repair as evidenced by window blinds have broken slats. IMMEDIATE: Director will have these replaced in the next week. The main building had a recent water leak that has been repaired and further repairs are scheduled during winter break. Water stains were present in ceiling tiles and light shielding throughout multiple classrooms, bathrooms, and hallways. Large ceiling bubble observed in the hallway restroom and temporarily covered cutout made for prior repair in toddler restroom. Corrective Action Plan IMMEDIATE: Finishing repairs are already scheduled during the center's holiday break. Director is also going to have the contractor check for active leaks at this time. PREVENTATIVE: Director will continue scheduling repairs as needed. A center may use portable fans if the fans are secured and inaccessible to children and do not present any tripping, safety or fire hazards. In the event air temperature in a center exceeds the 82 degrees Fahrenheit in the summer months because of evaporative cooler temperature limitations, it will be verified that cooling equipment is functioning, is being maintained, and that supplemental aides have been employed, such as, but not limited to: ceiling fans, portable fans, or portable evaporative coolers.
