The child(ren) in the Combination Class room(s) was/were left unattended when 2-year-old female left the group on 1/6/26(Approx 1-2 Minutes) and on 1/12/26(Approx 4 Minutes). Immediate: The director put a supervision support plan in place with the parent and educators on 1/12/26. The Director spoke with staff on 1/6/26. Staff were written up for both incidents. Water coming from a faucet is above 110 degrees Fahrenheit. On 4/30/24, the Director took immediate action by asking the Maintenace team to lower the water temperature. The Maintenace team stated that they would need to add a tempering valve for the sinks in the classrooms. On 5/6/24, the Director stated that tempering valves were being added to regulate the hot water temperature. Effective 5/6/24. the center will do monthly temperature checks on the water to ensure the temperature does not exceed 110 degrees.
