3A:52-4.3(a) · The children shall be supervised directly by a staff member at all times, including during outdoor activities, rest and sleep, and walking through hallways. Toileting procedures shall be supervised by a staff member as appropriate for the ages and developmental needs of the children.

Based on information, ensure that adequate supervision is maintained at all times throughout the center. Two named staff was not aware that a named child sustained an injury to his eye in their care.

Based on information, retrain all staff on: 1. The center's supervision policy; 2. The center's policies and procedures for incidents, including notifying parents of facial injuries within the required timeframes.
