At the time of inspection, a child was observed alone with a therapist in room 6. The therapist had gone for fingerprinting, however, the center did not have the results yet. The therapist did not have a cleared CARI with the center and had not completed the required orientation training. When discussed with administrative staff, the child and therapist were moved back into the child's classroom where there were staff members who had the required clearances. Provide an MOU stating that the center understands, and agrees, that children can not be left alone, at any time, with individuals who do not have a cleared CHRI and CARI under the center's license number, and who have not competed the required orientation training.
