A child was alone in the hallway bathroom with the door closed without supervision. 6-10-25: Ensure children walking through the hallways are supervised at all times. There was a child was dropped off and walking alone to her classroom. 7-21-25: Ensure that children are supervised at all times when walking through the hallways. A 5 year old child from room 5, was observed walking a child age 3, to room 9 without adult supervision. 7-21-25: In rooms 3 and 6 children were observed sitting on top of chairs and stacked chair piles without staff intervening. 7-28-25: Ensure that children coming in from the bus are supervised at all times. Staff sent 3 children inside to room 2 without ensuring that there were staff in room. 9-30-25: Ensure children coming off the bus are directly supervised at all times. 8 children were observed in room 5 with no staff present after coming into the building from the bus. 2 children were observed going into room 9 after coming into the building from the bus, when no staff were present in the room. Children were walking the hallways to classrooms without direct supervision after coming in the building from the buses.
