3A:52-7.6(b)(6) · (b) The center shall maintain on file a written record of each incident resulting in an injury as specified in (a) above. These records shall include the following: 1. The name of the injured child; 2. The date, time and location of the incident; 3. The name and address of the center; 4. The name of the person completing the report; 5.
