3A:52-4.3(a) · The children shall be supervised directly by a staff member at all times, including during outdoor activities, rest and sleep, and walking through hallways. Toileting procedures shall be supervised by a staff member as appropriate for the ages and developmental needs of the children.

Ensure staff are actively supervising children at all times. Children were observed drinking from each others sippy cups and staff were unaware.

3A:52-7.7(a)(1)(i) · Disinfectants shall be used as follows: the center shall disinfect those items specified in N.J.A.C. 3A:52-7.7(a)3 with a solution that shall be a commercially prepared disinfectant that indicates it kills bacteria, viruses, and parasites. This solution shall be used in accordance with label instructions.

Ensure staff remove dirty gloves and discard with the dirty diaper before putting on a clean diaper and redressing the child as to not cross contaminate.

3A:52-7.7(a)(1)(ii) · Disinfectants shall be used as follows: the center shall disinfect those items specified in N.J.A.C. 3A:52-7.7(a)3 with a solution that shall be a self-made solution consisting of one-quarter cup of household bleach to each gallon of water (one tablespoon per quart), which shall be prepared daily and placed in a labeled, sealed container.

On the day of the inspection observation revealed that staff was applying bleach and water and soap and water on the diapering table at the same time. Washing and disinfecting is a two step process and must be followed as such.
