75-03-09-27 · (5)-75-03-09-27. Effect of conviction on licensure and employment. 5. A provider shall submit an application for a fingerprint- based criminal history record check at the time of application and within five years from the date of initial approval and at least once every five years thereafter. The provider shall ensure that each staff member submits an application for a fingerprint-based criminal history record check upon hire and within five years from the date of initial approval and at least once every five years thereafter. The department may excuse an individual from providing fingerprints if usable prints have not been obtained after two sets of prints have been submitted and rejected. 75-03-09-22 · . Records.-2. The provider shall maintain the following records: a. The child's full name, birthdate, and current home address; b. Legal names of the child's parents, and current business and personal telephone numbers where they can be reached; c. Names and telephone numbers of individuals who may assume responsibility for the child if the individuals legally responsible for the child cannot be reached immediately in an emergency; d. A written statement from the parents authorizing emergency medical care; e. Names and telephone numbers of individuals authorized to take the child from the group child care; f.
