While doing an unannounced visit to discuss and accident that occurred, I went into one of the staff's rooms that was present at the time of the accident. The staff Savannah Close was in the infant room. There were no other staff in the room. She had 5 infants.

75-03-10-27 · (5)-Effect of conviction on licensure and employment. An operator shall submit an application for a fingerprint-based criminal history record check at the time of application and within five years from the date of initial approval and at least once every five years thereafter. The operator shall ensure that each staff member submits an application for a fingerprint-based criminal history record check upon hire and within five years from the date of initial approval and at least once every five years thereafter. The department may excuse a person from providing fingerprints if usable prints have not been obtained after two sets of prints have been submitted and rejected. If a person is excused from providing fingerprints, the department shall submit a request to the bureau of criminal investigation for a nationwide name-based criminal history record check.
