75-03-09-08 · (3ab)-3. The provider shall report to the department or its authorized agent within twenty-four hours: a. A death or serious accident or illness requiring hospitalization of a child while in the care of the group childcare or attributable to care received in the group childcare. b. An injury to any child which occurs while the child is in the care of the group childcare, and which requires medical treatment.

During the concern visit Andy Sandbeck (owner/operator) told this authorized agent a Serious Accident, Injury or Illness/Report was not done due to the child not going directly to the hospital. The report should have been completed within 24 hours of the parent notifying the facility the child was taken to the hospital as a result from the accident on the playground the day before.
