Upon investigating a concern, it was found that the owner has lacked in communicating personnel policies with the staff members. Some staff members did not have the knowledge of who was in charge during an emergency. Some staff didn't have knowledge that a situation was in progress or the degree of the situation. It was found that staff did not say anything to the person in charge when a child left his classroom unattended several times throughout the day.

During an unannounced visit, it was found a staff member sleeping while the children were laying down for nap. Some of the children were not a sleep yet. The staff had to be shook to be woke up.
