At 3:26 pm, the child entered the bathroom in the hallway as part of a full bathroom break before going to the gym. The preschool aide and substitute teacher had 12 preschool age children in their care at the time. At 3:31 pm, the preschool aide and substitute teacher took the children who were lined up in the hallway to walk to the gym. At the time, the child was behind the curtain in the doorway. The curtain is used to support a separate child when he uses the bathroom. The child was left behind unattended in the bathroom until 3:37 when he left the bathroom and went into the hallway. A different teacher noticed him and attended to him. He was briefly observed in the camera view at 3:35 before he went back behind the curtain in the bathroom. Heather, as the Site Coordinator of this location, was notified by the preschool aide at 4:33 pm via text message that the incident took place. The employee expressed her concern regarding the incident and how upset she was that the incident occurred. The child’s parent was working in the building as a substitute at the time and is the teacher who attended to the child. The child’s parent was upset at the situation but understanding. This timeline was created utilizing the camera system in the building. The preschool aide is our full-time employee. Neither employee was observed counting children or confirming that they had all children accounted for.
