on 4/6/26 an Unannounced visit was completed. Per disussion with CBCU Unit, it is being found that employee does not have a current results memo on file. She submitted her paperwork back in July 2024 but was closed unprocessed due to forms being rejected.

On 4/6/2026, I completed an Unannounced visit that is required once a ratio waiver is requested. When I arrived, there were 2 staff members present. I was told that Shyia stepped away for an emergency. Upon reviewing records/ratios, it was found that the two staff members were over ratio with a ratio of 2.66. During my visit, Shyia did return which put them back into ratio.
