The provider shall obtain a fire inspection prior to licensure and annually thereafter. Any inspection fees are the provider's responsibility. The provider shall have any code violations noted by the fire inspector corrected and shall file reports of the inspections and any corrections with the department. If the fire, safety, health, or sanitation environment appears questionable, the department may require the provider to obtain additional inspections at the cost of the provider. While reviewing through annual review paperwork, it was noted that Binx and Ryder were lapsed in their vaccinations. During a joint unannounced visit, the unvaccinated pets were seen around children multiple time. During a joint visit, during the walkthrough of the home, it was noted that Ronda has 3 cats and 2 dogs, while only 2 cats and 2 dogs records were uploaded in the system, the 4th cat was not - despite of age, the cat will either need to be removed from the childcare during childcare hours or needs to be vaccinated for rabies, and added to the portal.

Household members age twelve and older shall complete, and the provider shall submit to the department, a department-approved authorization for background check form at the time of application, relicensure, or upon obtaining residence at the location of the family child care.
While checking on a concern, there was a male in the basement without a background check completed. During the investigation we learned he was the husband to the childcare owner, and lives at the childcare when he is not on the road for work.
