75-03-10-27 · . Effect of conviction on licensure and employment.-5. An owner shall submit an application for a fingerprint-based criminal history record check at the time of application and within five years from the date of initial approval and at least once every five years thereafter. The owner shall ensure that each staff member submits an application for a fingerprint-based criminal history record check upon hire and within five years from the date of initial approval and at least once every five years thereafter. The department may excuse an individual from providing fingerprints if usable prints have not been obtained after two sets of prints have been submitted and rejected. If an individual is excused from providing fingerprints, the department shall submit a request to the bureau of criminal investigation for a nationwide name-based criminal history record check.
It was noted from CBCU staff that All 4 Learning had employed a staff member, EM without receiving the fingerprint memo, staff member had been working from 6/6/24 to 6/24/24 without a CBCU memo. EM cannot come back to work until a cleared memo comes back. Another staff member, DL was working with an expired Fingerprint memo (memo was dated 10/24/2018, and expired 10/24/2023) DL, cannot come back to work until a new/clear background check comes back.
