During and unannounced visit it was observed there were three staff members that stated they were sub staff for the day. Upon verifying their qualifications, I was unable to find two of the staff members listed in the system or listed in Growing Futures. While at the facility I was able to verify the CBCU date but not for Megan Millikin. The staff I was able to verify as a staff member for another facility was Abby Pink. Megan Millikin and Alexie Shelbie I could not find listed in the system or on Growing Future. Both staff are not list as emergency staff for this facility.

Email was sent by a staff stating the facility left a child at the school when they left for a field trip. Even though the parent did not sign the child in via ProCare at another facility. The other facility brought the child to the Independence site for the field trip. The child fell asleep on the rug and was left. The child was found by a school staff member. The school bus left the facility around 8:45am and the YMCA was called by the school at 9:48am.
