75-03-11 · .1-08.4(8)-All staff members shall be currently certified within ninety days of employment and prior to staff member having unsupervised access to children under care, in pediatric first aid by a program approved by the department.
During review of the documents submitted for the facilities annual renewal it was noted that staff did not have a Pediatric 1st aid certification from a department approved site with the first 90 days of employment.

75-03-11 · .1-08(1a)-The operator shall: a. Ensure that the director is present at the school-age childcare program at least sixty percent of the time that the program is open. If the operator has legal responsibility and the administrative authority over two or more school-age childcare programs, a director shared between two or more school-age childcare programs shall be present a combined total of sixty percent of the school-age programs' hours of operation.
A) Prior to my unannounced visit I reviewed the file and noticed the Director listed I know had left employment or Supervisor listed. I reached out to see who the Director and Supervisor were since I knew the director listed had left employment. On 10.17.24 I received an email stating who the Director and Supervisor were. When I went to my unannounced visit the Supervisor Maria and another staff were present. When I asked about the director both staff confirmed the director to be Taylor Filipek. Both the supervisor and the staff stated that Taylor had not been at the facility once since the beginning of school.

75-03-11 · .1-08(3)-The operator shall notify the department of any major changes in the operation of, or in the ownership or governing body of the school-age childcare program, including staff member changes.
I was not notified when the new director and new supervisor were hired. Both staff that were present during the unannounced visit stated the director was hired at the beginning of the school year. It is still unclear when his first day at the facility was. The supervisor told me her first day was 9.1.24. As of today, there is no director/supervisor change application in the portal. On 8.2.24 an email request was sent asking for an updated list of director and supervisor for all YMCA sites. We did not receive the updated list. 8.12.24 an email was sent requesting the Director/Supervisors for this licenser's sites. The email included this facility. 9.25.24 an email was sent with reminder regarding director/supervisors at multiple sites. Spreadsheet of the directors/supervisors again was requested. 10.3.24 a meeting was held with YMCA staff and licensing staff to go over licensing's expectations. 10.18.24 after receiving an email stating who the director/supervisor were for this facility I did a quick spreadsheet showing who the director/supervisor currently listed in the system are for all the YMCA programs. 10.21.24 after receive notification that Maria Johnson had been added to the portal a follow-up email was sent with a reminder that changes regarding the director/supervisor will need to be done via the director/supervisor change application.
