During the unannounced visit staff member (Alexis Jetvig) was asked who the director and supervisor were. She stated she was the supervisor, and Nicole Linman was the director. Upon verifying the supervisor's qualifications, it was noted the current director and supervisor were not listed. I later confirmed Nicole Linman is acting director and Alexis Jetvig is acting supervisor. Also, while verifying Alexis Jetvig qualifications, it was also noted that she was listed as a previous employee. The department was not notified that Alexis was a new hire.

During the unannounced visit made on 12/18/2024 the specialist was informed Alexis Jetvig is currently the supervisor of this program. As of today, supervisor qualifications have not been submitted for review
