During the visit it was observed in one of the rooms a nebulizer was placed in a sink with a medication bottle next to it. In another room there were medication bottles placed in plastic bag on the counter. Both medications were not properly stored and out of reach of children.

As staff qualifications were reviewed, it was noted that three staff members: Arrianna J., Chyanne M., and Lerynngies T. did not have a current SFN 375 transfer forms completed. A request was made for the current SFN 375 transfer form to be sent via email or uploaded to the system. Later this afternoon an email was received by the Criminal Background Check Unit stating the SFN 375 transfer forms for the 3 staff members had been rejected.
