75-03-08-24 · (1)(d)(2) -The provider shall ensure that infants sleep in a crib with a firm mattress or in a portable crib with the manufacturer's pad that meets consumer product safety commission standards.
Upon arrival it was noted there was an infant under 12 months of age. As we walked in, a 9-year-old brought out the infant from the first room. During the walk through of the home, it was noted, a crib or pack-a-play was not observed. The room the infant was in only had to mattresses on the floor.

During the concern visit, a container of Roundup was on the living room couch, there were also construction tools in the living room, and bedrooms accessible to children. Downstairs it was noted a box of moth balls and a pair of scissors were within reach of children.

75-03-08-14 · (2)-The family childcare must be clean and maintained to protect the health and safety of children. The family childcare and outdoor play area must be free of clutter, accumulation of refuse, standing water, unprotected wells, debris, and other health and safety hazards. Garbage must be regularly removed.
Upon review of the childcare it was noted there were two roles of chain link fence laying in the front yard, several metal posts with the concrete leaning against the front of the house by the front door. The licensing agents also noted large pickup parts and a jack on the ground within reach of children. In both bedrooms it was noted pillows were being used to keep air conditioners in the windows.

75-03-08-14 · (8)-The provider shall ensure that exterior play areas in close proximity to busy streets and other unsafe areas are contained or fenced, or have natural barriers, to restrict children from those unsafe areas. Outdoor play areas must be inspected daily for hazards and necessary maintenance.
During the concern visit the following hazards were found outside where children play: a broken swing set, broken toys, garbage/debris, and a couch setting half on/half off the swing set causing a potential hazard to children.
