75-03-09-09 · .4-The provider of a group child care shall ensure that the group child care is sufficiently staffed at all times to meet the child and staff ratios for children in attendance, and that no more children than the licensed capacity are served at one time.
An unannounced visit was conducted on 6/17/23 at 840am. At the time of the visit there was 1 staff with 10 children present which represented 1.93 points. 1 staff can care for 1.34 points.

75-03-09-18 · .3-Minimum sanitation and safety requirements. The provider shall ensure that the group child care building, grounds, and equipment are located, cleaned, and maintained to protect the health and safety of children. Routine maintenance and cleaning procedures must be established to protect the health of the children and the staff members.
A visit was conducted on 4/19/2023. The program was found to have dirt, garbage and leftover dried food from the previous day throughout the kitchen/table area. There was several locations of dried food, under the tables, around the high chairs and by the cooking area. What was found was uncooked broken spaghetti noodles along with prepared spaghetti and corn that was dried. Some of the food was stuck to the floor and some was loosely laying. This was found where children, including children under 2 years old were actively playing. There was also a large portion of dried mud and dirt chunks by the back door. The mud chunks were both in and out of the infant area. The infant area had several large chunks of mud that could have easily been consumed by the infant in care. The chunks of mud in the infant area were found to be approximately 2-3 feet into the infant area with it being around and on infant toys and equipment. The program had excessive dirt and garbage throughout the program along walls, edges and randomly throughout the program that appeared to have been there for more than the current day and previous day.
