A provider shall submit an application for a fingerprint-based criminal history record check at the time of application and within five years from the date of initial approval and at least once every five years thereafter. The provider shall ensure that each staff member submits an application for a fingerprint-based criminal history record check upon hire and within five years from the date of initial approval and at least once every five years thereafter. The department may excuse an individual from providing fingerprints if usable prints have not been obtained after two sets of prints have been submitted and rejected. If an individual is excused from providing fingerprints, the department shall submit a request to the bureau of criminal investigation for a nationwide name-based criminal history record check.

Was at 1.81. Had Megan call parents to pick up children. Looked over numbers for tomorrow and she would be at 1.29. Starting Monday, a three year-old will not be attending and she will be in ratio. Also reminded her to complete Safe Sleep tonight. Received an email that the parents were notified and pulling out their child until she has room.

Today when I arrived, Megan was by herself with a ratio of 1.43. Nancy did show up about 10 minutes after arriving. Discussed with Nancy the correction orders, the process and I printed them off for her to hang. She said she was checked out and not here 60% of the time.

Staffing requirements.-7) When there is a mixed-aged group, the number of children in each age category is multiplied by the corresponding ratio number, converted to decimal form, and carried to the nearest hundredth. To determine the number of staff members responsible for caring for or teaching children necessary at any given time, numbers of staff members for all age categories are added, and any fractional staff member count is then rounded to the next highest whole number whenever the fractional staff member count amounts to thirty-five hundredths or more. If lower than thirty-five hundredths, the fractional amount is dropped. No more than four children under the age of eighteen months per staff member are allowed in any mixed-aged group.

During the review of the paperwork it was determined that Linda's fingerprints where not processed. Nancy was notified on January 20, 2023. During the visit I discussed with Nancy that we only had preliminary fingerprints on file and to contact the CBCU department. Nancy was notified at that time that Linda did not have fingerprints but still worked after that information was shared with Nancy.

Legal names of the child's parents, and current business and personal telephone numbers where they can be reached; c.
