Name of Operation: KIDS LEARNING ACADEMY, INC. Facility ID: 35000156 Consultant: FELICIA FAISON Operation Type: Center Case Number: Visit Date: 6/30/2026 Number Present: 36 Completed Date: 6/30/2026 Age: From 0 To 9 Total Minutes: 195 Time In: 09:00 AM Time Out: 12:15 PM Time In: Time Out: List to Use: Center Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of my unannounced visit today was to monitor compliance with child care requirements applicable to a routine unannounced visit. G. Hunter, administrator, arrived after the visit began and assisted. Susan Fuller, Licensing Supervisor, was present and assisted with the visit. Currently, this center operates with a Four-Star license issued on November 6, 2018, earning five (5) points in the Education Component, six (6) points in the Program Component and one (1) Quality point. The last annual compliance visit was conducted on December 3, 2025. The last sanitation inspection was completed June 17, 2026, earning a Superior rating. The last fire inspection was completed September 25, 2025, and the facility was approved for care on first shift only. Children were observed circle time, activity center play and napping. Observed interactions between children and teachers were nurturing in nature. The following violations of child care requirements were documented. Violation Number Comment Rule 705 Equipment and furnishings were not sturdy, stable and free of hazards. There was a chair with a ripped cushion and exposed stuffing in space #4. .0601(c) 847 Parent's medication authorization did not include required information. A child with CVS Acetaminophen in space #4 did not have a completed medication authorization form available. 10A NCAC 09 .0803(4)(6-9) The violations documented above must be corrected immediately. A signed and dated letter of compliance can be mailed or emailed to me and must be received by July 14, 2026. If mailing, you must submit two (2) signed and dated copies. My mailing and email addresses are noted at the bottom. The letter of compliance must detail how violations were corrected. Please be aware any written information you submit regarding the correction of the violation is legal documentation. Therefore, it is important that all submitted information is accurate and truthful. If it is determined that submitted information was inaccurate or willfully falsified, an Administrative Action, including Revocation of the facility’s license could be issued. STAFF RECORDS You, G. Hunter, and L. Taylor noted that no new staff have been hired since the annual compliance visit. Therefore, staff files were not monitored. INDOOR EQUIPMENT & FURNISHINGS During the visit I observed a foldable adult chair in space #4 that had a ripped cushion. The rip was on the back of the cushion and exposed the stuffing which could have been torn off in small pieces. Upon notifying you, you removed the chair. Therefore, this was documented as corrected during the visit. MEDICATION A child in space #4 has a container of CVS children’s acetaminophen on-site. There was no completed and signed medication authorization form on file for the child’s medication. ABCMS PROVIDER PORTAL As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Providers were required to create a Business NCID, unless they already had one, and complete Provider Portal training in Moodle. I observed that the center has a roster in the ABCMS center. TECHNICAL ASSISTANCE: Technical assistance discussed with regarding changing the child proof locks in space #4 for the counter beneath the refrigerator. It appears as though the current locks are no longer catching which may make the contents accessible to children. I recommend power washing the equipment and storage containers on the playgrounds to remove any/all built up dirt, spider webs, etc. that can accrue after time and the recent rain storms. Please review the Southwestern Child Development Commission for training that meets on-going training hours and CEU requirements. REMINDERS: The Clean Water for Carolina Kids program expanded to the Clean Classrooms for Carolina Kids program. This is a legislatively mandated effort to address lead and asbestos hazards in North Carolina public schools, licensed child care centers, and licensed family child care homes. Rules 10A NCAC 41C .1001 through .1007 require all programs to be free of lead and asbestos hazards. RTI is coordinating all the lead in water, paint and asbestos testing and remediation. Currently operating child care facilities are required to test for lead in paint and asbestos once unless the more is deemed necessary. After reviewing the website, I noted that you have completed your second round of lead water testing as of April 2024 and are not due again until 2027. However, the website (https://www.cleanwaterforuskids.org/en/carolina/) lists the program as “enrollment started” for the lead paint and asbestos testing. You stated they told you they would contact the center and schedule a time to come out and complete this testing. QRIS RATED LICENSE TRANSITION We discussed the education standards requirement that fifty (50) percent of staff meet the education star level to meet it. At the 4- and 5-star level lead teachers and administrators must complete a training on implementing the Creative Curriculum and Teaching Strategies GOLD as they are your chosen curriculum and formative assessment. I recommend you reach out to FGV Smart Start as they offer training on the Creative Curriculum. Child care licensing requirements are established to ensure a safe and healthy child care environment. It is your responsibility to maintain compliance with all applicable laws and rules. The best way to do so is to periodically review the child care laws and rules. The current rule book dated July 1, 2025, is available at the DCDEE website https://ncchildcare.ncdhhs.gov/. There are also modules reviewing the new Child Care Rules available on Moodle, which can be found on the Division website. The last fire inspection was completed November 1, 2024 and the facility was approved for care on first shift only. Children were observed engaging in activity center play. Observed interactions between teachers and children were positive and nurturing in nature. The following violations of child care requirements were documented. Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. There was no current activity plan posted in space #6 for reference. GS 110-91(12); .0508(a) 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. A container of Aquaphor in space #6 expired August 2024. .0803(12) 1882 Medication authorization, giving the caregiver standing authorization did not meet the specifications in rule. The medication authorization form for the Aquaphor in space #6 was expired. .0803(6)(a-i); .0803(7)(a-g); .0803(8)(a-d) The violations documented above must be corrected immediately. FACILITY PROFILE This facility is owned by Kids Learning Academy, Inc. The corporation is listed as current/active in the North Carolina Secretary of State corporation’s database. I reviewed contact information for the owner and the facility with you, G. Hunter, and you confirmed that the information listed with DCDEE remains correct. CHILDREN’S RECORDS I monitored ten (10) percent or more of children’s records and no violations were documented. STAFF RECORDS I monitored ten (10) percent of staff records as well as both new staff records. No violations were documented. MEDICATION While monitoring space #6 I observed a container of Aquaphor that expired August 2024 for an enrolled child. The medication authorization form for the Aquaphor also expired in August. There was no current lesson plan posted for the group enrolled in space #6 as it was dated for last week (December 2nd-6th). Expired medication must be sent home or discarded once it expires to ensure it is not administered to the child. Current lesson plans must be posted so staff, parents and visitors may reference it to know what the children are learning or if they are helping implement the activities. TECHNICAL ASSISTANCE & REMINDERS: Continue to monitor metal play equipment on the playground for signs of peeling or chipping paint. In order to keep them in good repair if this begins the items should be removed or repaired/re-painted. Also, routinely monitor the fencing to ensure it remains at least four (4) feet high. Licensing fee invoices were emailed in November 30th. Ensure that your email address remains current and once received that the payment is made online, on the Division website, by December 31, 2024. Please be reminded that failure to pay the license fee is a violation of General Statutes 110-90(1a) and will result in a penalty assessment, accrued interest, and issuance of an Administrative Action. ENVIRONMENTAL RATING SCALES You were reminded that the hold harmless state has been extended. Therefore, programs that were in cohorts 1 and 2 do not have to proceed with the Environmental Rating Scale assessments (ERS).
