This program currently operates with a Five-Star Star License issued June 11, 2018 earning seven (7) points in staff education, six (6) points in program standards and one (1) quality point. The center continues to meet enhanced space and enhanced ratios. The last sanitation inspection was completed on March 4, 2025 with a “Superior” rating. The last fire inspection was completed on February 26, 2025 and was approved for daytime care only. The last annual compliance visit was completed on February 3, 2025. K. Bell, Center Manager, was present and available to assist. Thirty-five (35) children were present age ranging from one (1) to four (4) years of age. Children were observed in free choice play, teacher directed activities, and daily routine transitions such as toileting and hand washing. The center is owned by Choanoke Area Development Association of North Carolina, Incorporated. This corporation was found to have a Current/Active Status under the NC Secretary of State website. I used the Annual Compliance Monitoring Checklist for Child Care Centers and the Child Care Center Item Number Listing as a basic monitoring tool to assess compliance with all applicable child care requirements pertinent to this facility. Ten percent of staff records and ten percent of children’s records, and all program records were monitored. All program records and children’s records were found to be in compliance. One (1) violation was documented for staff record keeping/trainings. Due to internet connectivity issues, a one-page, handwritten visit summary was reviewed and left with you at the completion of the visit. A detailed, computer-generated visit summary will be emailed to you. The following violations of child care requirements were documented during the visit. Violation Number Comment Rule 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. First Aid training for A. Boyce expired 1/12/2026. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. CPR training for A. Boyce expired 1/12/2026. .1102(d) Child Care Licensing Requirements are established to ensure a safe, healthy, and developmentally appropriate environment for all child care facilities. Therefore, it is important for you to be knowledgeable of all licensing requirements that apply to your facility. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. It is your responsibility to maintain compliance with the applicable laws and rules; always whether they have been monitored or specifically discussed with you in the past. The best way for you to ensure that you are meeting all requirements is to periodically review the childcare laws and rules on the Division of Child Development and Early Education (DCDEE) website. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before January 27, 2026, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. I used a checklist as a basic monitoring tool to assess compliance with all applicable childcare requirements pertinent to your program. Due to connectivity issues a handwritten/typed visit summary was reviewed with you at the conclusion of the visit. A computer-generated visit summary will be emailed to you. The following violations of child care requirements were documented today. Daily routine transitions were also observed such as toileting and hand washing. Lunch consisted of tuna, whole grain crackers, backed beans, lettuce, tomato and milk. The center is owned by Choanoke Area Development Association of North Carolina, Incorporated. This corporation was found to have a Current/Active Status under the NC Secretary of State website. I used the Annual Compliance Monitoring Checklist for Child Care Centers and the Child Care Center Item Number Listing as a basic monitoring tool to assess compliance with all applicable child care requirements pertinent to this facility. LEAD IN WATER/PAINT AND ASBESTOS Public schools and licensed child care facilities in North Carolina are eligible to participate in the funding mechanisms to cover the costs of testing for lead in water and inspecting for asbestos and lead-based paint hazards beyond the May 1, 2024 deadline listed in 10A NCAC 41C .1002(a). Due to the recent impacts of Hurricane Helene and other natural disasters, this deadline has been revised statewide, and facilities that have not yet enrolled should complete the online process by May 31, 2025. The Clean Classrooms for Carolina Kids Program is providing free facility-wide testing and inspections without any out-of-pocket costs. Due to the volume of testing and inspections, completion of the online enrollment process in the program is considered temporary compliance with the testing and inspection requirement and will allow facilities to participate in the funding mechanisms. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS on an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. PATHWAYS TO THE PROFESSION The Division of Child Development and Early Education (DCDEE) prioritizes increasing career pathways for future ECE teachers. However, after lengthy careful consideration DCDEE has reluctantly decided it will not bring back the Early Childhood Equivalency Exam due to concerns about academic integrity. We make this decision to ensure fair and equitable opportunity for all aspiring early childhood teachers, uphold minimum educational standards, and keep staff and children in licensed child care environments safe. Note, other Pathways to the Profession of Early Childhood Teacher still include: • EDU 119 – Introduction to Early Childhood Education: This 12-week course introduces the foundations of culturally responsive, equitable and inclusive early childhood education. Available at many N.C. Community Colleges with online, evening, or in-person options. Upon successful completion of EDU-119, the student earns the North Carolina Early Childhood Credential and qualifies to be a lead teacher in a child care center. • Test-Out Option for EDU 119: An early childhood professional who has gained knowledge through work and lived experience and/or professional development may be able to receive Credit for Prior Learning by taking a challenge exam for EDU119. Community College for testing information. • EDU 3119 – Intro to Early Childhood Education: This new short-term (8-week) continuing education offering is designed to prepare students to work in a childcare facility. Students learn how to design developmentally appropriate and inclusive lesson plans and activities for young children in alignment with NC Foundations for Early Learning and Development and state regulations. At the end of the course, students will have obtained a qualifying letter which provides eligibility for employment in a N.C. child care facility. Contact a local N.C. This program currently operates with a Five-Star Star License issued June 11, 2018 earning seven (7) points in staff education, six (6) points in program standards and one (1) quality point. The center continues to meet enhanced space and enhanced ratios. The last sanitation inspection was completed on January 27, 2025 with a “Provisional” rating. The last fire inspection was completed on January 30, 2024 and was approved for daytime care only. The annual fire inspection has not been completed and is delinquent. The last annual compliance visit was completed on March 14, 2024. P. Hill, Cafeteria Manager, was present and available to assist. Twenty-two (22) children were present age ranging from two (2) to five (5) years of age. Children were observed in free choice play, and teacher directed activities, and lunch. Daily routine transitions were also observed such as toileting and hand washing. Lunch consisted of tuna, whole grain crackers, backed beans, lettuce, tomato and milk. The center is owned by Choanoke Area Development Association of North Carolina, Incorporated. This corporation was found to have a Current/Active Status under the NC Secretary of State website. I used the Annual Compliance Monitoring Checklist for Child Care Centers and the Child Care Center Item Number Listing as a basic monitoring tool to assess compliance with all applicable child care requirements pertinent to this facility. Ten percent of staff records and ten percent of children’s records, and all program records were monitored. All children’s records monitored were found to be incompliance. Two (2) violations were documented for staff record keeping, and two (2) violations were documented for program record keeping/annual inspections. Due to connectivity issues a handwritten/typed visit summary was reviewed with you at the conclusion of the visit. A computer-generated visit summary will be emailed to you. The following violations of child care requirements were documented today. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy five percent. It is your responsibility to maintain compliance with the applicable laws and rules; always whether they have been monitored or specifically discussed with you in the past. The best way for you to ensure that you are meeting all requirements is to periodically review the childcare laws and rules on the Division of Child Development and Early Education (DCDEE) website. This program currently operates with a Five-Star Star License issued June 11, 2018 earning seven (7) points in staff education, six (6) points in program standards and one (1) quality point. The center continues to meet enhanced space and enhanced ratios. The last sanitation inspection was completed on January 8, 2024 with a “Superior” rating. The last fire inspection was completed on January 30, 2024 and was approved for daytime care only. The last annual compliance visit was completed on April 18, 2023. B. Beale, Center Director was present and available to assist. Twenty-eight (28) children were present age ranging one to five years of age. Children were observed in breakfast, free choice play, and teacher directed activities. Daily routine transitions were also observed such as toileting and hand washing. Breakfast consisted of waffles, strawberries, and milk. The center is owned by Choanoke Area Development Association of North Carolina, Incorporated. This corporation was found to have a Current/Active Status under the NC Secretary of State website. I used the Annual Compliance Monitoring Checklist for Child Care Centers and the Child Care Center Item Number Listing as a basic monitoring tool to assess compliance with all applicable child care requirements pertinent to this facility. Ten percent of staff records and ten percent of children’s records were monitored. All staff records monitored were found to be incompliance. Three (3) violations were documented for staff record keeping. One (1) violation was written for safe outdoor environment due an active ant mound in the outdoor play area. Due to connectivity issues a handwritten/typed visit summary was reviewed and left with you at the conclusion of the visit. A computer-generated visit summary will be emailed to you. The following violations of child care requirements were documented today. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. An active fire ant mound was observed in the outdoor play area for Preschool children, near the swings. 10A NCAC 09 .0601(a) 1314 Emergency information did not name childs health care professional. The Health Care professional and Hospital Preference were not included in the emergency information for A. Harrell. .0802(c)(2) 1320 Children's records that include an application for enrollment, medical and immunization records, and permission to seek emergency medical care was not on file for each child. The medical record for A. Harrell was not on file for review. GS 110-91(1);.0302(d)(2); .0304(g) 1321 Medical exam or health assessment record was not on file before or within 30 days after admission. The medical record was not on file for A. Harrell within 30 days after the child's admission to the facility. GS110-91(1) Child Care Licensing Requirements are established to ensure a safe, healthy, and developmentally appropriate environment for all child care facilities. Therefore, it is important for you to be knowledgeable of all licensing requirements that apply to your facility. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy five percent. It is your responsibility to maintain compliance with the applicable laws and rules; always whether they have been monitored or specifically discussed with you in the past. The best way for you to ensure that you are meeting all requirements is to periodically review the childcare laws and rules on the Division of Child Development and Early Education (DCDEE) website. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before March 28, 2024, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is accurate documentation. If any information provided in the letter is not accurate, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. The compliance history score will be emailed to you along with the computer-generated visit summary. TECHNICAL ASSISTANCE/ CONSULTATION CHILDRENS RECORD KEEPING On child’s record monitored, A. Harrell, was missing the health care professional and hospital preference on the application. We discussed this emergency medical information is required to be on the application in case of a medical emergency with the child. The same record did not have the physical examination within thirty (30) days of enrollment. The physical examination was not on record for the child. We discussed that every child is required to have a physical examination on file within thirty (30) days of the child enrolling into the facility. SAFE OUTDOOR ENVIRONMENT An active fire ant mound was observed on the Preschool playground, by the swings. We discussed that if one of the children were to fall on the ant mound, they could be bit/stung by the ants. You spoke with maintenance during the visit and the mound was treated. Visit this website for the most up-to-date child care forms, resources related to child care, and new child care rules/laws. LEAD IN WATER/PAINT AND ASBESTOS The Clean Water for Carolina Kids program is expanding to the Clean Classrooms for Carolina Kids program. This is a legislatively mandated effort to address lead and asbestos hazards in North Carolina public schools, licensed child care centers, and licensed family child care homes. Participation allows facilities to meet all rule requirements for identified lead and asbestos hazards, as required by law. Rules 10A NCAC 41C .1001 through .1007 require all programs to be free of lead and asbestos hazards. RTI is coordinating all the lead in water, paint and asbestos testing and remediation. There are webinars that child care providers can watch to get instructions. Go to https://www.cleanwaterforuskids.org/en/carolina/ for more information. Currently operating child care facilities are required to test for lead in paint and asbestos once unless the consultant notes a concern during a visit and refers the program to environmental health. All existing licensed programs must complete the application summary for their program regarding lead paint and asbestos by May 1, 2024. Rule 15A NCAC 18A .2816 requires currently operating child care centers to test water initially and every three years. Most of you have completed your initial testing and now it is time for your three-year retest. The last annual compliance visit was completed on April 18,2023. C. Robinson, Staff Development Coordinator, was present and available to assist. Twenty-five (25) children were present age ranging from one to five years of age. Children were observed in free choice play, teacher directed activities and outdoor play. Daily routine transitions were also observed such as toileting and hand washing. All interactions between caregivers and the children were positive and nurturing. The center is owned by Choanoke Area Development Association of North Carolina, Incorporated. This corporation was found to have a Current/Active Status under the NC Secretary of State website. I used a checklist as a basic monitoring tool to assess compliance with all applicable childcare requirements pertinent to your program. The following requirements were monitored during today's visit: Staff/Child Ratio, Supervision, Specialized trainings, Storage of Hazardous Products, Storage of Medication, Adequate/Approved Space, General Safety, Staff Records, Program Records, and License Posted and Permit Restrictions. Four (4) new staff records were monitored today and found to be in compliance. Program records were monitored and a violation was written for non-compliance. Due to connectivity issues, a hand-written visit summary was reviewed and left with you. A computer-generated visit summary will be emailed to you. The following violations of child care requirements were documented today. Violation Number Comment Rule 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. One (1) Epi Pen was observed in Space #5/6 that had expired on October 19, 2023. .0803(12) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. The last drill was documented February 2023. .0604(u);.0302(d)(8) 1835 The medical action plan was not updated on an annual basis or when changes to the plan were made by the child's parent or health care professional. The medical action plan for the child's Epi Pen expired on November 14, 2023 .0801(b) 1882 Medication authorization, giving the caregiver standing authorization did not meet the specifications in rule. TECHNICAL ASSISTANCE/ CONSULTATION PROGRAM RECORDS A shelter in place/lockdown drill was not practiced and/or documented. Shelter in place/lockdown drills are to be practiced and documented every three (3) months in accordance with child care rules. The last drill was documented February 2023. CHILDREN’S RECORD KEEPING/MEDICATION One (1) Epi Pen that had expired on October 19, 2023, was observed in Space #5/6. The medication authorization and the medical action plan for the Epi Pen expired on November 14, 2023. All medications that have expired should be returned to the parent or discarded once the medication has expired and/or the authorization has expired. The medical action plan should be updated on an annual basis or when changes are made to the plan by the parent or health care professional. REMINDERS The sanitation inspection for the facility will expire on December 6, 2023. I encourage you to contact your local sanitation inspector to ensure the inspection will be completed before the current inspection expires. The fire inspection for the facility will expire on January 31, 2024. I encourage you to contact your local fire inspector to ensure the inspection is completed before the current inspection expires. More information will be provided regarding your programs rated license assessment timeframe. 2023 ANNUAL LICENSE FEES There are a few changes to the Annual License Fee process this year: • All invoices will be emailed, and no paper copies will be sent through the U.S. Postal Service. • All invoices will be emailed by December 1, 2023 and will include the invoice number and total amount due. • All payments are due no later than December 31, 2023 Payments must be submitted online only through the Division’s website using a Mastercard or Visa debit or credit card, or by e-check. Providers will need their Facility ID number and their 2023 Invoice number to complete the payment process. Annual license fees do not apply to state or public-school operated child care facilities, or to religious sponsored facilities operated pursuant to a Notice of Compliance. Failure to pay the license fee is a violation of NCGS § 110-90(1a) and can result in a revocation of your child care license. In addition, unpaid license fees will be assessed a 10% late fee and accrued interest as established by NCGS § 147-86.23.
