Your program currently operates with a three-star license, issued 1/17/18, earning 4 points in the education component, 2 points in the program standards component, and 1 quality point for the programmatic option of infant capacity restricted to no more than two children under one year of age. The last annual compliance visit was conducted 5/16/24. The family child care home's compliance history was reviewed with the operator. The program’s compliance history was ninety nine percent as of 4/22/25. Corporation Status: Per your statements and according to the NC Secretary of State's website, this facility is not currently owned or operated by a corporation. Please contact me prior to any changes regarding the ownership of this facility. We reviewed your facility contact information listed on the Division of Child Development and Early Education (DCDEE) website and all information is current. If a new resident is to join your ho Operator’s Statements of Responsibility and Verification of Required Information for Operator and Additions Caregivers forms were reviewed during the visit. You stated you are to currently the only household member. You greeted me at the door. You were present with two children in care. You sat on the floor with the children and play with wooden blocks in the living room area. The group later transitioned to the child care space, where children participated in music and movement, with instruments. You talked with the children, asking open-ended questions, counted, and talked about the children’s day. Lunch was observed and consisted of chicken nuggets, fries, apples, and milk. The following violations were observed. Violation Number Comment Rule 930 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years from the date of issuance). The child care provider did not complete and submit the required forms to complete a criminal background check prior to the January 15, 2025 expiration of the previous qualification letter. G.S. 110-90.2 & .2703(n) & (o) 1704 Summary of the NC Child Care Law was not given to each child's parent, guardian, or full-time custodian before the child was enrolled in the home and/or signed statement was not on file. Signed acknowledgement of the receipt of the Summary of the NC Child Care Law was not on file for the child enrolled on 9/16/24. GS 110-102 1808 Signed statement did not include the date the operator's safe sleep policy was given and explained to the parent/guardian. G.S. 110-90.2 & .2703(r) 1990 Infant feeding plans did not include the required information, including but not limited to the type and amount of milk, formula and food, and the frequency of feedings, and/or was not available for reference. An infant child enrolled on 9/16/24 did not have an infant feeding plan of file for review during the visit. .1706(i) 2023 Operator and/or staff who work with children, did not complete health and safety training as part of on-going training so that every five years, all the topic areas were covered. The operator did not complete trainings on six (6) of the eleven (11) required topic areas as a part of the on-going training. .1703(d)(2) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. On or before 5/7/25, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. You stated that you were tracking the expiration and believed it expired January 2026. Children's records consist of various documentation such as a child's medical and immunization history, emergency medical care information and parental permission to participate in specific activities and documentation of acknowledgement of facility policies and procedures. Today the file for the child enrolled on 9/16/24 did not include a signed safe sleep policy, infant feeding schedule, nor a acknowledgement of receipt of the Summary of NC Child Care Law. You stated that you have all parent complete the enrollment packet and was uncertain how the forms were misplaced. Today we discussed your current practices for receiving and filing children’s records. You may consider using a signature page that includes the acknowledgement of multiple policies on one page instated of the multiple pages you are currently using. Health and Safety Trainings - Child care providers who have direct responsibility for the care and oversight of children must complete health and safety training to comply with the NC Child Care Rules. Training can be taken in-person by contacting your local partnership or on-line through the DCDEE Moodle Portal. Additionally, health and safety training must be completed as part of on-going training so that every five years, all required topic areas will be covered. The five-year recertification was due on or before April 15, 2025. You have completed training in five of the required topic areas since the previous certification in April 2020. You shared that you were focused on having CPR/First Aid and ITS-SIDS trainings, and the other topics slipped by you. Today, we discussed your current tracking methods for training. Set reminders to help ensure the trainings are completed in the required timeframe. Also you may completed one to two trainings each month to complete them over time, verses all at once. ABCMS Provider Portal Training- As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. Effective February 2024, the process of notifying the Division is captured in ABCMS. You shared with me that you were unaware of the training or that it was required. You will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. Once the training has been completed and access has been given, you must verify your facility roster to ensure current household members are noted on the roster. This information should be updated in ABCMS on an ongoing basis as household members change or if staff members hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. Rated License Information: The program currently operates with a three-star license, issued 1/17/18. You completed the rated license application today and signed the assessment for two component license form which displayed the program standard points, education standard points and quality points. Based on the information noted above, your program earned 4 points in the Education component. Program Standards – Operational policies have previously been reviewed and are on file. The provider does not want to have the ERS completed at this time. You have begun a Three-Month self-study and self-assessment using the FCCERS-R has been completed. Please notify me upon completion. Quality Point – Provider is continuing to meet the Quality Point with Program Options of no more than two Infants under age one will be served, therefore your program earned 1 Quality Point.
